Tobacco Cessation Tool Kit
The American College of Chest Physicians (ACCP) is a
medical specialty society of physicians, surgeons, and allied health
professionals who specialize in diseases of the chest.
The ACCP Tobacco Cessation Tool Kit is now available to help physicians,
nurses, and cessation counselors aid their patients in the quitting process.
The comprehensive kit incorporates everything from identifying users and
assessing their risks and level of
addiction, to providing motivational advice, pharmacotherapy information,
and follow-up and relapse prevention techniques. This toolkit is based on

effects, and contraindications).
- US Public Health Service Guidelines

- Assessment of Tobacco Risk Factors Questionnaire
- Fagerstrom Test for Nicotine Dependence

- Two Patient Education brochures developed by the ACCP to (1)
motivate the user to want to quit and (2) provide a how-to guide to
tobacco cessation
- Quit Contracts
- List of national, state, and local Referral Sources
- Rewards for success
- Consultation Reports
- Additional patient education materials
The Tobacco Cessation Tool Kit also provides educational information on
the biology of nicotine addiction, treating nicotine addiction as a chronic
